COORDINATION
OF CARE: The Specialty nurse will coordinate and
refer to our PT, OT, ST, and MSW as the physician requests. We will also refer
to community services as the patient may need.
WE
PARTNER WITH YOU: Specialty’s nurses work in
partnership with the physician and office staff to coordinate care and to meet
the needs of the patient. Our nurse's will develop a close working
relationship with you that engenders confidence and trust in the nursing care
given at home. They will contact you with updates when the patient’s condition
deteriorates, new needs are identified, or changes in the medical Plan of Care
are needed.
