Care Management
Our Case Management nurses will provide preventative monitoring for your
loved ones who want to remain at home. Planning by an experienced
professional nurse enables our clients to stay at home longer by promoting
safety, prevention of complications, and coordination of all needed services by
one person. This service includes:
- NURSING CARE: Comprehensive assessment of physical condition,
including medications and environment modification needs by a Registered
Nurse. A plan of care will then be developed by our experienced Registered
Nurse.
- MONITORING:
Ongoing monitoring of physical condition, identifying changes, and coordinating
your needs with the physician.
- MD
APPOINTMENTS: Coordination and scheduling of medical appointments,
including transportation
- LAB
TESTS: Our skilled nurses and draw laboratory tests ordered by
your physician, in your own home.
- MEDICATIONS: Medication set up and continual monitoring for side effects,
interactions, and compliance problems, will be provided by our nurses.
- FOOT
CARE: Our specialty trained Foot Care nurses will assist in
trimming thickened and difficult to trim nails. Our foot care included
soaking, trimming, filing, and a wonderful foot massage.
- MEDICAL
SUPPLIES: Our nurses will assist in obtaining all medical
supplies and medical equipment that is needed to safely and comfortably stay at
home.
- THERAPY:
Our therapists provide rehabilitation services to
help maintain independence and safety while staying at home
- BATH
ASSISTANCE: Our Certified home health aides provide personal care
assistance, as needed, to help you remain in your own home.
